2017 Scholarship Recommendation Form

This form must be completed by a school official and submitted by Friday, March 10, 2017. A transcript of the applicant's grades must also be submitted by Friday, March 10, 2017 and can be sent via email to pat.daly@musicbiz.org or mailed to Music Business Association Attn: Pat Daly, 1 Eves Drive, Suite 138, Marlton, NJ 08053. If a student has been out of high school for more than one year, an employer or college official recommendation letter can be considered in lieu of the Recommendation Form completed by a high school official.
